It is working fine now.  I also added a non-maskable interrupt button and a real-time clock.  The 2nd board revision is here:

I have thought it was possible to build a $10 68000 SBC using 10cm*10cm pc board, obsolete parts (old CPLD, 16meg SIMM, and NMOS 68000), and cheap serial flash, but the resulting board was too spartan and it was still more than $10.  I compromised and added a CF-IDE, socketed the major components, add a 7-seg display and end up with a $20 SBC that's CP/M-68K capable.  Looking back, I'm happy with the compromise.
